**There are not prescribed forms to register a Limited Liability Partnership (LLP). When registering an LLP, the application must contain, or confirm:

The name of the LLP
That the LLP carries on the practice of an eligible profession
That there is either one or two representative partners in Alberta
That the registered address and, if present, the address for service by mail is in Alberta
The originating jurisdiction - for extra-provincial limited liability partnership registrations
That the professional endorsement has been provided
That the proof of status from the home jurisdiction is present - for-extra provincial liability partnership registrations
